Patients accepted by a Family Physician at our clinic may book an appointment online. We are not a walkin clinic for the general public.

If you have been referred to a physician at our clinic, please follow the instructions you were given by our clinic in order to book your appointment.

What to know before your appointment

  • Just like in person, they may be running a few minutes late 
  • For telephone appointments, their caller ID may show up as BLOCKED or UNKNOWN, so please be sure to pickup these calls at the time of your appointment. 
  • Please do not enter the clinic if you are experiencing cough/cold symptoms. If you are experiencing any respiratory symptoms before your in-person appointment, please contact the office so that we can ensure you don’t inadvertently infect other patients.

Book an Appointment With Our Health Management Nurses

Appointments can be booked with our Health Management Nurse for the following reasons:

In-clinic appointments:

  • Chronic Disease Management Check-Ins (Diabetes, Hypertension, High Cholesterol) 
  • Pre-operative Assessments 
  • Ear flushing 
  • Removal of staples, sutures
  • Immunizations 
  • Cognitive Testing 

Telephone appointments:

  • Review of lab/tests
  • Referrals to Community Services 
  • Follow up on Emergency Room visits
  • Chronic Disease Management Check-Ins (Diabetes, Hypertension, High Cholesterol) 
  • Lifestyle Management (diet, weight management, setting goals) 
  • Mental Health check-ins/connections 
  • Frailty check-ins
  • Smoking Cessation
