Book an Appointment With Our Health Management Nurses

Appointments can be booked with our Health Management Nurse for the following reasons:

In-clinic appointments:

  • Chronic Disease Management Check-Ins (Diabetes, Hypertension, High Cholesterol) 
  • Pre-operative Assessments 
  • Ear flushing 
  • Removal of staples, sutures
  • Immunizations 
  • Cognitive Testing 

Telephone appointments:

  • Review of lab/tests
  • Referrals to Community Services 
  • Follow up on Emergency Room visits
  • Chronic Disease Management Check-Ins (Diabetes, Hypertension, High Cholesterol) 
  • Lifestyle Management (diet, weight management, setting goals) 
  • Mental Health check-ins/connections 
  • Frailty check-ins
  • Smoking Cessation

Patients of Dr. Abhari and Dr. Easterbrook can book an appointment with a nurse by clicking the button below. Patients of Dr. Asplin can book an appointment with a nurse by calling the office.

NOTE: Registered Nurses cannot provide prescription refills, diagnostic tests, or laboratory requisitions.

 All in-clinic appointments must be made via reception.

What to know before your appointment

  • Just like in person, they may be running a few minutes late 
  • For telephone appointments, their caller ID may show up as BLOCKED or UNKNOWN, so please be sure to pickup these calls at the time of your appointment. 
  • You can manage your appointments online through the patient portal.
  • If you are having a virtual visit with Dr. Abhari or Dr. Easterbrook, you will be sent a unique code and link 5-10 minutes before your visit, via SMS and/or email.
  • Please do not enter the clinic if you are experiencing symptoms of COVID-19. If you are experiencing any COVID-19 symptoms before your in-person apppointment, please contact the the office so that we may take the next approprate steps. 

COVID-19 symptoms include: cough, sore throat, runny nose, nausea/vomiting, diarrhea, headache, no taste/smell, and shortness of breath.
